(MIB) is a central database of medical information. There are approximately 15 million Americans and Canadians on file in the MIB's computer database, and over 750 insurance firms use the services of the MIB to obtain information about life insurance and individual health insurance policy applicants. The MIB does not have a file on everyone, but if your medical information is on file, you will want to be sure the information is correct. website to download and print the form you will need to complete and mail in.
